A report into how a woman was dragged along the platform by a train when her arm got trapped has been published.

The incident happened at Bushey Station on on March 26 at 9.24pm, when the woman became stuck.

The Safety Digest has been published by the government's Rail Accident Investigation Branch, so lessons can be learnt to prevent this from happening again.

The train was the 20:59 hrs West Midlands Trains service from Tring to Euston.

The victim's forearm got caught in the external door of the train and she had to walk, then run, beside it to avoid being pulled off her feet.

She began shouting 'stop, stop' as the train reached speeds of 9mph.

It had travelled 27 metres - 13 seconds - before being brought to a stand by the train's conductor.

The passenger remained upright, but sustained a minor injury to her forearm.

The conductor said that after passengers had left and boarded the train at the station, he stood on the platform observing the doors close.

But station CCTV shows the woman involved in the accident hurrying onto the platform as this was happening, placing her forearm between the closing doors.

The conductor urged the passenger to stand clear of the train - something the woman says she did not hear. He then sent the driver a signal to let him know it was clear.
