New documents have been added to the brough council's planning portal for the property at 147 North Approach, part of the town's Kingswood Estate.
They seek permission for a part single and part two-storey rear extension, as well as a hip-to-gable loft conversion, while also seeking retrospective permission for previously carried out works to the driveway.
Watford Borough Council refused an initial application last year for a seven-person HMO over "poor connection to public transport" and question marks over the quality of accommodation.
The latest proposal, which does not specify how many bedrooms it seeks to include this time, will be bad news for the 39 previous objectors, including one long-time resident who branded it a "potential monstrosity".
The proposed building works are largely identical to the rejected proposal.

Four people have already objected to the new plan, with one writing: "This part of Watford is characterised predominantly by traditional family housing, with the majority of properties being three-bedroom semi-detached homes occupied by single households.
"The proposed HMO represents a significantly more intensive form of occupation than is typical of the surrounding area and is therefore out of character with the established residential nature of the neighbourhood."
The applicant has not submitted a supporting statement on this occasion, although has previously said that turning this property into a HMO would "result in a net gain of an identified housing need and would provide a high-quality living environment for future occupiers".
An appeal against the initial refusal was shot down in March after the planning inspectorate agreed with the borough council, stating that the property "would not be suitable" for use as a HMO.
Watford Borough Council has set a target decision date of July 21 for the revised proposal.
